IMPORTANT: THIS AGREEMENT IS A LEGALLY BINDING CONTRACT UNDER THE LAWS OF THE REPUBLIC OF TÜRKİYE.

This Affiliate Program Agreement (“Agreement”) is entered into between MJB Care (“Company,” “we,” “us”) and you (“Affiliate,” “you”).

This Agreement governs your participation in the MJB Care Affiliate Program (“Program”).

1. Nature of MJB Care (Mandatory Disclosure)

MJB Care is a health services organization and medical facilitator based in Türkiye.

We:

• Connect patients with licensed clinics and doctors

• Provide coordination, assistance, guidance, and support

We do NOT:

• Perform medical treatments

• Act as a healthcare provider

 Treatment outcomes depend on the clinic and doctor selected.

2. Affiliate Program Structure (Tier System)

MJB Care operates a tier-based commission structure.

Affiliates earn based on performance level and volume of patients referred.

Level 6 — Bronze

1–10 Referrals

• 5% Commission

Level 5 — Silver

11–29 Referrals

• 5% Commission
• €1000 Bonus

Level 4 — Gold

30–69 Referrals

• 5% Commission
• €2000 Bonus
• 24K Custom Gold Ring

Level 3 — Platinum

70–99 Referrals

• 5% Commission
• €4000 Bonus
• Flight Ticket
• 10g 24K Gold

Level 2 — Emerald

100–499 Referrals

• 5% Commission
• €7000 Bonus
• Family Vacation
• 50g 24K Gold
• 3 Shares in MJB Care

Level 1 — Diamond

500+ Referrals

• 5% Commission
• Surprise Bonus
• Annual Family Vacation
• 100g 24K Gold
• 5 Shares in MJB Care
• Additional Surprise Reward

Notes:

• Referral = completed and paid patient treatment
• Bonuses are awarded upon reaching each tier
• MJB Care reserves the right to verify referrals and adjust rewards

 Tier Rules:

• Tier upgrades are based on:

  • Number of completed patients

  • Performance consistency

• MJB Care reserves the right to:

  • Adjust tiers

  • Reassign levels

  • Review performance at any time

3. Commission Eligibility

A commission is earned only if:

1. The patient uses your Affiliate Link/Code

2. Must not be registered with another affiliate previously

3. The patient pays for treatment.

4. Payment is successfully received.

   >Commission is calculated from the total paid cost of his medical treatment.

 Not Eligible:

• Self-referrals

• Misleading promotions

4. Pricing Transparency & Ethical Standards

Affiliates must communicate that:

• Patients should verify clinics, doctors, and materials

• MJB Care provides:

  1. Verified providers

  2. Transparent guidance

  3. Authentic materials with proof

  4. Lifetime support

 Strictly Prohibited:

• Guaranteeing results

• Making false medical claims

• Misrepresenting pricing or treatments

5. Anti-Scam & Patient Protection Position

MJB Care operates with a patient protection model.

We actively aim to protect patients from:

• Hidden fees

• Fake or low-quality materials

• Unethical upselling

• Misleading clinic practices

– Affiliates must reflect this honest positioning, not exaggerate or mislead.

6. Payments

• Paid 1-7 days after confirmation of sale.

• Payments made after:

  • Treatment completion

  • Payment clearance

You must provide:

• Valid identity

• Payment details

7. Tracking & Attribution

• Tracking is based on Affiliate Links/Codes

• MJB Care is not responsible for:

  • Broken links

  • Cookie blocking

  • User behavior

8. Intellectual Property

You may use:

• MJB Care brand assets

• Approved marketing materials

You may NOT:

• Modify branding

• Claim to represent MJB Care officially

9. Confidentiality

You must protect:

• Patient data

• Internal business information

This obligation continues after termination.

10. Termination

We may terminate immediately if you:

• Mislead patients

• Damage reputation

• Violate laws or this Agreement

 Upon termination:

• Affiliate rights are revoked

• Commissions may be forfeited

11. Limitation of Liability

MJB Care is not responsible for:

• Medical outcomes

• Clinic performance

12. Governing Law (Türkiye)

This Agreement is governed by the laws of the Republic of Türkiye.

All disputes shall be resolved in:

— Istanbul Courts and Enforcement Offices

13. Independent Relationship

You are an independent affiliate, not:

• Employee

• Agent

• Legal representative

14. Updates to Agreement

We may update this Agreement at any time.

Continued participation = acceptance.
